A single proposal form may be completed for all courses in a sequence proposed for WIC designation; otherwise, a separate proposal form must be completed for each course proposed for WIC designation. Completed proposal forms, with attachments and the signature of your department chair or program head, should be submitted to the Academic Affairs Curriculum Subcommittee for consideration.
